Lipid-lowering therapy for prevention of ventricular tachyarrhythmias.

چکیده
In this issue of the Journal, Mitchell et al. (1) report that lipid-lowering therapy (LLT) was associated with a 36% relative risk reduction (RRR) in all-cause mortality in patients with ischemic heart disease (IHD) enrolled in the Antiarrhythmics Versus Implantable Defibrillator (AVID) trial. This rivals the benefit observed for implantable cardioverter defibrillators (ICDs) over amiodarone therapy (RRR 38%) (2) and is greater than the effect of LLT observed in patients with IHD in the Scandinavian Simvastatin Survival Study (4S) (RRR 30%) and in the Long-term Intervention with Pravastatin in Ischemic Disease (LIPID) trial (RRR 22%) (3,4). This striking effect could be

Statins: Newer Roles Including Lipid Lowering Therapy
Statins are most effective pharmacologically agents to decrease total plasma cholesterol level by competitive inhibition of HMG-Co A reductase. Apart from the well-known LDL and cholesterol lowering effect, statins have been postulated to exert other beneficial effects also called pleotropic or non-lipid effects. Better understanding of various pleotropic effects of statins has prompted a new s...

Ninety-six patients with recurrent, drug-refractory tachyarrhythmias were treated with amiodarone for 8.0 + 7.5 months (range 1 day to 27 months): 77 for recurrent ventricular tachycardia or ventricular fibrillation (VT/VF), two for complex ventricular ectopy, and 17 for supraventricular tachyarrhythmias. The actuarial incidence of successful amiodarone therapy was 52 + 7% at 12 months and 28 +...
متن کاملThe effect of statin therapy on ventricular tachyarrhythmias: a meta-analysis.
The objective of this study was to assess whether statin therapy is associated with a reduction in ventricular tachyarrhythmias. Statins have been shown to be beneficial beyond their cholesterol-lowering effects. These pleiotropic effects have been implicated in the protection against atrial fibrillation and the reduction in appropriate implantable cardioverter-defibrillator therapy in patients...
متن کاملIs prophylaxis the best use of the ICD?
When Michel Mirowski initially conceived of the implantable defibrillator the aim was to protect the patient at risk of sudden arrhythmic death. In the late 1960s, when Mirowski and co-workers projected the development of an automatic implantable defibrillator, out-of hospital resuscitation was a rare event. However, the out-of-hospital resuscitation initiatives led by investigators from Seattl...
متن کاملPrevention of inappropriate therapy in implantable cardioverter-defibrillators: results of a prospective, randomized study of tachyarrhythmia detection algorithms.
OBJECTIVES The purpose of this randomized study was to investigate the performance of single- and dual-chamber tachyarrhythmia detection algorithms. BACKGROUND A proposed benefit of dual-chamber implantable cardioverter-defibrillators (ICDs) is improved specificity of tachyarrhythmia detection. METHODS All ICD candidates received a dual-chamber ICD and were randomized to programmed single- ...
